(MAP) is a Maryland statewide program offered in all counties. MAP provides information and assistance to adults, individuals with disabilities and caregivers. It can become overwhelming for persons who are searching for options to plan for current and future long term services and supports. We offer referrals for community based services to help people who live alone or with others to potentially avoid nursing home stays (Some programs have eligibility guidelines).
MAP staff can provide links to services such as: home-delivered meals; transportation; home modifications; energy assistance; legal resources and state health insurance program assistance (Medicaid and/or Medicare). Our staff will help you explore your options, assess your level of care needs and develop an options counseling plan which identifies the services needed for current and futures planning. Ask our MAP staff about the community waiver programs for which you eligible for to assist with personal care, personal emergency response systems etc.
